Waist drawstring on jacket is longer than 3 inches when the waist is expanded to its fullest width, and there are toggles and knots at the end of the un-tacked drawstring. This can cause the drawstring to catch on protrusions (such as bus doors or handles), potentially causing injury or death if the person is dragged by the vehicle. See Report 04-040 11/26/03		Zipper Pulls Sold Jan. 01 - Nov. 03 American Greetings Corp Cleveland, Ohio	This accessory item consists of a 2-inch plastic blue or purple butterfly-shaped photo holder that can be attached to a zipper. There are four items in a package. The packaging has a pink/hot pink cardboard backer. The product could separate and detach from the zipper, posing a choking hazard to young children. These jackets are made of blue or olive nylon fabric and are decorated with assorted patriotic patches on the sleeves, chest and back. The hood is bordered with synthetic brown and tan fur. These jackets have hood drawstrings. See Report 04-032 11/13/03		Multicolored Sidewalk Chalk Sold March 03 - July 03 Agglo Corporation imported by Target Corporation Hong Kong	The sidewalk chalk is packaged in plastic that is molded to five sticks of chalk and a cardboard backing that is labeled "Double Dipp'n Fun." The multicolored sidewalk chalk contains high levels of lead, posing a risk of poisoning to young children. The Wisconsin Department of Health tested the chalk and identified the lead in the chalk. The recalled Lightning PAC Scooters and MX3 Mini Bikes are battery powered ride-on toys designed for children ages six years and older. The motor control circuits can malfunction causing the scooters and mini bikes to continue to run after the power or throttle button is released, posing a risk of injury to children. These bunk beds have openings between the guardrails and between guardrails and the end structures that are too large. Only ³Pack Œn Play² portable play yards with raised changing tables are part of this recall. When children are placed in these portable play yards when the changing table is still in place, they can crawl under and lift the table up. If this occurs, a child¹s head and neck can become trapped between the changing table and the play yard rail, causing a strangulation hazard. The pacifiers are available in both silicone and latex formulations in the following colors: powder blue, red, purple, and pink. The name "Kroger" appears on the package, along with the words "Pacifiers," "Latex," "Silicone," "0-6 months," or "6-18 months." These pacifiers fail federal safety tests, come apart, and can pose a choking hazard to infants and small children.	Return the pacifiers to the store where purchased for a full refund. A plastic replica of a balloon attached to the book can detach, posing a choking hazard to young children. The recalled baby walkers are made with a plastic frame supported by six or eight wheels on the bottom. The walkers are blue, pink or bright green with a padded seat and an activity tray. The walkers will fit through a standard doorway and are not designed to stop at the edge of a step. Babies using these walkers can be seriously injured or killed if they fall down stairs. The recalled "Sandy Claws" Swim Trainer is a red and yellow colored fabric crab with eight stuffed legs and two eyes covering a styrofoam buoyancy float. The nylon body strap on the swim trainer can detach or tear from the flotation device and release a child into water, posing a serious drowning hazard to young children. The metal pendant has assorted symbols on one side. The necklace's pendant contains high levels of lead, posing a risk of poisoning to young children. The recalled toys are battery-operated, multi-colored toy jet airplanes measuring about 9 inches in length and width. A clear plastic cover over the cockpit exposes a pilot with his hand on the controls. A label on the jet's wings reads "FALON" (Falcon misspelled). Small parts of the toy jet can detach, posing a choking hazard to young children. The recalled Recycling Truck Puzzle has nine wooden pieces that form a green recycling truck, three workers, a container for plastics, a bundled set of newspapers, and a recycling bin. One of the puzzle pieces (a stack of newspapers) poses a small parts choking hazard to young children. The sets were sold separately with 12 villagers per set and as a part of the Happyville Special set that included a set of 35 Happyville Blocks. The head can detach from the body of the villagers, posing a choking hazard to young children. The chairs are steel outdoor chairs with a white frame and a pink or blue sling fabric seat and back. The chairs measure about 9.5 inches in height. Malibu, Calif	The "Spit Smatter" spray comes in a pressurized can and emits a colored foam based on one of six brands: "Original Smatter," "Blueberry Smatter," "Banana Cream Smatter," "Lemon Meringue Smatter," and "Fatter Smatter." The aerosol cans can forcefully break apart, posing a risk of serious injury to nearby consumers. See Report 03-148 6/25/03		Extended Recall of Infant Car Seats/Carriers Sold March 95 - Jan. 00 Dorel Juvenile Group Inc. Columbus, Ind.	The extended recall includes the Cosco Arriva and Turnabout infant car seats/carriers to address a problem with the carry handle. When the seat is used as a carrier, the plastic handle locks can unexpectedly break or release from the carrying position, causing the seat to unlatch or flip forward. When this happens, an infant can fall to the ground and suffer injuries. Please see the original report for model numbers.	Consumers should continue to use the Arriva and Turnabout as a car seat but should stop using it as a carrier immediatly and contact Dorel Juvenile Group to receive a free repair kit. See Report 03-146 6/19/03		Crib Mobile Toys Sold Dec. 99 - Dec. 01 Fisher Price East Aurora, N.Y.	These are Sparkling SymphonyTM Mobile crib toys that attach to the crib side rail and play music and lights that are activated by remote control. Four star and moon characters are suspended from the mobile, which turns. I batteries used in the mobile leak, the caustic liquid can seep out of the toy's battery compartment, posing a risk of chemical burns to babies.	Consumers should contact Fisher Price at 800-357-9460 with the date code to determine if their mobile is part of the recall. A free repair kit containing a seal for the battery compartment is available for recalled products.
See Report 03-138 6/5/03		Children's Art Supply Sets Sold Aug. 02 - March 03 Creative Kids Inc. Monsey, N.Y.	The 87-piece art supply sets were sold in solid wood grain boxes. The set contains crayons, markers, scissors, water and oil paints, brushes, and a mini-cutter with razor blades. The razor blades pose a laceration hazard to young children.	Consumers should remove the mini-cutters and razor blades from the set and return them to Creative Kids to get a free pack of colored paper clips. The "Tiffany" and Josephine" model cribs are made of solid natural wood with a chest of drawers attached to the footboard. The slats on the drop-side rails can become loose or detach. A child's head can get caught in the space left by loose or missing slats, presenting an entrapment hazard. The tumbler cup is constructed of translucent plastic in the shape of a bear with the words "BearistaTM Bear printed on the front. When the flexible straw on the cup is chewed, small pieces of plastic can detach, posing a choking hazard to young children.	Parents and caregivers should stop young children from using these cups immediately, and return the cups to any Starbucks location for a free beverage of any size. The dive sticks are soft plastic tubes that have character heads and feet. Children can fall or land on these sticks in shallow water and may suffer impalement injuries. Included in the recall are "Always Crib,""Crib 4 life,""Set 4 life," and Crib-2-College." The Three hinges along the fold-down drop gate can crack or break and allow babies to have their fingers pinched.	Consumers owning these Baby's Dreams cribs should call Baby's Dream at 800-835-2742 to receive a repair kit for hinges and/or latches or for specific instructions for examining their latches.

Two small metal screws that hold the toy "stall doors" in place can come off, posing an aspiration or choking hazard to young children. The end piece of the drumstick handle can break off, posing a choking hazard to young children. Additionally, the screw at the end of the drumstick can loosen and detach, posing a choking hazard. The toy, which is designed to encourage crawling babies to pull themselves up and stand, can tip over during use and strike falling babies in the head, face or neck, resulting in injuries. The toy chest's lid support hinges can fail, allowing the lid to fall onto a child's head, neck, fingers or hands. In addition, spaces at the end of the hinge on the lid can cause pinch, crush or laceration injuries to children's fingers.	Consumers should stop using the Fiddlesticks immediately and return them to their Kindermusik educator for a free replacement. The 10-inch plush bear is white and dressed in a hat and jacket in one of the following colors: red, blue, hunter green, pink, purple, light green, light blue and white. The 10-inch snowman doll is dressed in a hat and jacket that are blue and pink. Buttons on the jacket of these toys can be pulled off, posing a choking hazard to young children. The Woody doll's clothing has buttons that can detach, posing a choking hazard for young children. The zipper box may detach, allowing the zipper slide to detach. The small parts, if detached, could pose a risk of choking. The plastic snaps can detach, posing a choking hazard to young children. The boxed sets included in the recall have children's characters on the front of the box, a colored plastic handle and plastic snaps.	Consumers should immediately cut off the lid of the box with the plastic snaps and throw away the rest of the box. The recalled toy is a small plush bug with two springy black antennae with orange round fabric ends. The bug has a blue stuffed round face with a red musical nose that blinks when pressed. The body of the bug is segmented in various colors and patterns. See Report 03-085 2/27/03		Combo Baby Tubs/Step Stools Sold Jan. 99 - Feb. 02 The First Years Inc. Avon, Mass	When used as a tub, babies' body parts can be pinched if the product's footrest is not fully extended so that it clicks into place firmly. CPSC and The First Years have received 20 reports of babies being pinched while using these tubs, including one bruising of a baby boy's genitalia and 10 reports of abrasions to toes and feet.	Consumers should not use the bathtub until they receive revised instructions on the use of the tub to prevent the pinching hazard. See Report 03-074 1/23/03		Repair Gerry® and Evenflo® Portable Wood Cribs Sold Jan. 91 - Dec. 02 Hufco-Delaware Company and Evenflo Company	If the hardware used to assemble the crib is not tight, the mattress support platform and mattress can fall to the floor. This poses a risk of injury to young children in the crib. There have been 41 reports of mattresses falling through portable wood cribs. Of these incidents, 17 children suffered bumps, bruises or scratches.	Consumers should stop using these portable wood cribs immediately, and call (800) 582-9359 anytime for a free upgrade kit that provides additional support for the mattress platform.

The "Founding Bear" is about 19-inches long, and the "Founding Bear II" is about 18-inches long. The nose of the stuffed bear can be pulled or twisted off, posing a choking hazard to a young child.	Consumers should take these stuffed bears away from young children immediately. See Report 03-067 1/9/03		Children's Soap Making Kits Sold March 98 - Nov. 02 Pace Products Inc. Apopka, Fla.	CPSC has received three reports of burn injuries from the heated soap, including a 6-year-old girl who received burns to her hand. The soap kits were sold under the name 'Soap Making for Kids." The soap may get too hot when heated in the microwave oven and leak from the tray mold posing a burn hazard to children.	Consumers should take these soap kits away from children immediately and contact Pace Products at (800) 541-7670 between 8 a.m. and 5 p.m. ET Monday through Friday to receive instructions on returning the soap kits for a refund.
